Ayushman Bharat represents the flagship national health initiative launched by the Government of India to fulfill the constitutional vision of Universal Health Coverage as articulated in the National Health Policy, 2017. Formally inaugurated on September 23, 2018, by Prime Minister Narendra Modi from Ranchi, Jharkhand, the program marks its eighth anniversary of nationwide operations in 2026. Designed as an integrated, dual-pillar healthcare delivery mechanism, Ayushman Bharat comprehensively restructures the public medical ecosystem by bridging preventive grassroots wellness with financial protection against catastrophic tertiary hospitalization expenditures.
The first foundational component of the initiative is the Pradhan Mantri Jan Arogya Yojana (PM-JAY), celebrated as the world's largest government-financed health assurance program. PM-JAY provides a completely cashless and paperless hospitalization benefit of up to five lakh rupees per family per year for secondary and tertiary medical care across a nationwide network of empaneled public and private hospitals. Initially targeting the bottom forty percent of the socio-economically vulnerable population identified through the Socio-Economic and Caste Census (SECC) 2011, the scheme experienced a historic milestone expansion in 2024, when the Union Government extended universal health insurance coverage to all senior citizens aged seventy years and above across India, irrespective of income or socio-economic criteria, under the Ayushman Vay Vandana card.
The complementary second pillar of the program focuses on primary health infrastructure through the operationalization of over one hundred and seventy thousand Ayushman Arogya Mandirs (formerly designated as Health and Wellness Centres). Upgraded from rural sub-centers and primary health posts, these centers deliver comprehensive primary healthcare, maternal and child clinics, free essential medicines, diagnostic screenings for non-communicable diseases, and wellness activities like Yoga. Supported by the digital infrastructure of the Ayushman Bharat Digital Mission, which has generated hundreds of millions of Ayushman Bharat Health Account (ABHA) IDs for interoperable electronic health record integration, the program has saved vulnerable households tens of thousands of crores in out-of-pocket medical expenses.
